Ingrown toenails didn't go away with surgery, help?

I've had two surgeries, one was to burn off the skin around and the other was to remove a part of the toenail growth system with a lazor. The first treatmeant did nothing and the second worked for a few weeks and then the ingrowns were back. They are always filled with a smelly puss that oozes and has to be cleaned every day. Often there is bleeding. I need a remedy that will help them (I've tried cotton, tooth picks, ingrown filers, surgery, letting them grow until the skin falls off, soaking and ointments of every sort.)
